Hy-Vee Health Exemplar Care provides primary care and urgent care through a membership model. Members receive easy scheduling with same-day or next-day availability, longer appointments focused on listening, and 24/7 access to a provider. Memberships include unlimited primary and urgent care visits, preventive physicals, chronic condition management, vaccines, routine and annual labs, EKGs, and select generic medications dispensed in the clinic. Transparent monthly pricing makes care predictable, with referrals to specialists at negotiated cash rates when needed. Care is designed around access, time, and your long-term health.
